(Published in The Norman Transcript November 7, 2016, 1t) CERTIFICATE OF NEED NOTICE Santa Fe Place, LLC applied to the Oklahoma State Department of Health (OSDH) for approval to establish a new sixteen (16) bed Immediate Care Facility for Individuals with Intellectual Disability (ICF/IID) located at 1000 C, South Santa Fe, Moore, Oklahoma 73160. Great West, LLC wish to establish a sixteen (16) bed ICF/IID facility. Great West, LLC has entered into a lease agreement with Santa Fe Place, LLC a domestic limited liability company. Santa Fe Place, LLC is owned eighty-five percent (85%) by William Forrest White, five percent (5%) by Janice Ann Talley, five percent (5%) by Josh White and five percent (5%) by Mark White, with DeNellda Jones listed as a person with authority in Santa Fe Place, LLC. The capital cost is $840,000.00. The application was filed under the state certificate of need laws, and is available for inspection at the OSDH Building, 1000 NE 10, Oklahoma City. Anyone desiring to participate in the review process must file a written notice of intent to participate, and file written evidence or argument.
