Case No.FA-2014-32 SECOND AMENDED NOTICE OF HEARING TO TERMINATE PARENTAL RIGHTS OF UNKNOWN FATHER AND KNOWN MOTHER, TO ALLOW ADOPTION WITHOUT CONSENT OF UNKNOWN FATHER AND KNOWN MOTHER, AND TO DETERMINE THAT UNKNOWN FATHER NEVER HAD PARENTAL RIGHTS AND ORDER AND NOTICE OF FINAL DECREE HEARING THE STATE OF OKLAHOMA, TO: Katrina Nichol Euell, Natural Mother and Unknown Father You are notified that Laveda Carol Ware has filed a Petition before the above named Court, praying that she be allowed to adopt the minor named in the caption hereof without your consent, and for adoption. On the 9th day of January, 2015, an Application was filed in this Court by the adoptive parent seeking an order of this Court to permit the adoption of without the consent of the unknown father and known mother and to terminate rights of unknown father and known mother and to determine that unknown father never had parental rights Joshua Dean Euell Ware, a male child born on May 16th, 2011, in Cleveland County, Oklahoma to Katrina Nichol Euell. Petitioner has alleged in the Petition that said child was born out of wedlock and that no person has acknowledged paternity of said child nor taken any action to legally establish claim to paternity of said child nor exercised parental rights or duties over said child. The said unknown father has failed to contribute to the support of the mother of the child to the extent of his financial ability during her term of pregnancy and up until the present time and that the unknown father has not provided for medical care for the birth mother and her child during the pregnancy and up until the present time and as a result failed to exercise parental rights and duties over the child. Further, said unknown father, has never had any contact with the child. Petitioner has alleged in the Petition that said child is eligible for adoption without the consent of the natural mother and that her rights should be terminated as the natural mother, Katrina Nichol Euell, has never paid child support on the above named minor child nor has she maintained significant contact with the child for 12 out of the last 14 months prior to the filing of the adoption Petition. A hearing on the Application to determine whether your consent to the adoption is necessary is set for 9:15 o'clock a.m., on the 25th day of April 2016, in the District Courtroom No. 2S, in the County Courthouse of Cleveland County, in the City of Norman, State of Oklahoma, before The Honorable Stephen W. Bonner. The Final Decree Hearing on the Petition is set for 9:15 o'clock a.m., on the 16th day of May 2016, in the District Courtroom No. 2S, in the County Courthouse of Cleveland County, in the City of Norman, State of Oklahoma, before The Honorable Stephen W. Bonner.
